BORROWER’S AGREEMENT
TERM: September 1, 2019-August 1, 2020
I have read the policies and procedures of TLC (on back) and agree to comply with them. Please know that I understand it is a privilege to use TLC. I will clean up after myself and children, take care of the items that I borrow, returning them promptly. I agree to pay any fees caused by the loss or breakage of library items while they are checked out to my family.
By signing this document I agree to always use best practices when choosing age appropriate equipment and toys for my child/dren.

POLICIES AND PROCEDURES
Membership
Any resident or staff member of Hopkins School District 270 or any resident of another school district attending ECFE classes at Harley Hopkins Family Center may use TLC. Each person must sign a borrower’s agreement before borrowing an item from TLC. A borrower’s agreement is valid from the date of submission to the office until July 31st.
Checkout
Please keep children in the lounge area, not in the library storage area behind the gate.
Each item is checked out for two weeks.
There is a six (6) item limit per two week period.
Follow the check out procedures posted at the checkout desk.
Please return items on time so that others may have a chance to use it.
Check-in
Returned items should be left in the bin next to the door.
Please Do Not check in your own item.
If you have lost or damaged an item please leave a note with your name, details of what happened, and an e-mail address or phone number on the desk in TLC.
Fines and Fees
Membership fees are $10.00.
Replacement of lost or destroyed items $5.00.
Checks should be made payable to ISD 270 and taken to the front/main office.
TLC Parent Lounge and Play Area
Please pick up after yourselves and help your children pick up after themselves.
Please make sure to remember that no children are allowed behind the gate.
There are toys and books are provided for your children’s use while in TLC/Family Lounge Area. The toys for check-out (numbered toys) are not meant for play during your family’s stay in the TLC/Family Lounge Area. Toys waiting to be checked back in should not be played with in the TLC/Family Lounge Area.
TLC is open August- July.
